You will need to provide our office with a certified copy of the party's birth certificate as proof of the correct name or date of birth. We will also need a notarized request from both parties stating what is to be amended on the marriage certificate. You will need to provide our office with a certified copy of the decree of divorce from the county. We will also need a notarized letter from the either party, or both stating what needs to be changed. View an Example of a Notarized Letter (PDF).
Since that item was already amended once, it will now require a court order to change again. Once the court order is completed, you must submit a certified copy of that court order to our office for the change to be made.

The fee to amend a divorce certificate is $20. This does not include the charge for a certified copy.
