To file a complaint concerning abuse, neglect or exploitation or any other patient or client care issues in a *medical care facility or provider regulated by the Kansas Department of Health and Environment (KDHE), Health Facilities Program, Acute and Continuing Care and or certified by the Centers for Medicare and Medicare (CMS), contact the Kansas Department of Aging and Disability Services (KDADS), as outlined on this page.
*Facilities include Hospitals (Acute Care, Specialty or Private Psychiatric); Critical Access Hospitals; Ambulatory Surgical Centers; Rural Health Clinics; End Stage Renal Disease facilities; Home Health Agencies; Hospice providers; Intermediate Care Facilities for Individuals with Intellectual Disabilities; Birthing Centers; Transplant Centers; Comprehensive Outpatient Rehabilitation Facilities; Mobile X-ray and Outpatient Physical Therapy providers.

If you are a *facility or provider listed on this page and wish to self-report a complaint concerning abuse, neglect or exploitation or any other patient or client care issue occurring in your workplace, please complete the Reporting Form (PDF) and FAX it to KDADS at 785-296-0256.
What Happens Next?
KDHE contracts with KDADS Hotline services for the management of complaint intakes for facility types we license or certify. Once the intake has been filed, KDHE is notified to initiate the investigation process.
