Once you have reviewed all the available resources on this website and have decided to move forward with the implementation of the Kansas Perinatal Community Collaborative model implementing the March of Dimes Becoming a Mom® curriculum, you will need to have your lead agency sign and submit the Memorandum of Understanding (MOU). While not a legally binding agreement, this MOU provides your agency's commitment to implementing the model and curriculum per the Kansas design as described throughout this website. Upon receipt and review of this MOU. The Kansas Department of Health and Environment may provide your agency with access to all implementation resources developed to support your community with implementation. We appreciate your partnership and adherence to the MOU requirements that support program fidelity across the state of Kansas.
If your community is interested in implementing, after reviewing all the resources on this website, please complete the Readiness Survey and Memorandum of Understanding linked above, and send it in via email.
