Communications Flowchart
The communications flowchart included in this document may be used by all health departments and should be modified accordingly to fit the needs of your agency and county plans, including Local Distribution Site (LDS) plans if applicable. Includes development of a communication pathway (aka flowchart) with management and command locations and support agencies.

COOP Activation Scenarios & Decision-Making Matrix
This matrix is designed to assist the Administrator or designee with determining whether to activate the COOP plan and/or the health department’s Incident Command System (ICS). The decisions on whether to activate the COOP plan and/or the ICS and under what conditions must be made by your agency. Thus, the health department must review this matrix and modify it according to fit your plans, needs, and capabilities.

COOP Reconstitution Team
This list may be used by larger health departments that have enough staff to designate a COOP Reconstitution Team to oversee the reconstitution process, which may include rebuilding or relocating operations. For smaller health departments, all staff members will be part of the reconstitution. In that case, the existing staff call-down roster will be used and it is not necessary to complete this spreadsheet. Smaller health departments may choose to exclude this spreadsheet from their COOP plan.

COOP Kit Inventory
The inventory list for equipment to relocate to an alternate worksite may be modified as needed to fit the needs of your health department. The following items listed in this spreadsheet are suggestions of things to take to an alternate worksite and should be amended as needed.

Cyber Security Annex to Emergency Operations Plan
This Annex assists in providing information about the cybersecurity process in coordination with Information Technology (IT) and Emergency Management (EM) personnel to identify, protect, detect, respond and recover from a cyber security incident affecting the healthcare environment.

Delegations of Authority
You may organize your Delegations of Authority by the essential functions, or you may choose to establish Delegations by program within the health department.

Devolution for County Health Department
Devolution is the capability to transfer statutory authority and responsibility for essential functions from an agency's primary operating staff and facilities to another organization's employees and facilities. Devolution planning supports overall COOP planning and addresses catastrophic or other disasters that render an agency's leadership and staff unavailable or incapable of performing its essential functions from either its primary or alternate facilities.

Essential Functions for County Health Department
Determine which functions of your health department must be operational within 30 days of a COOP event. Essential functions may include routine tasks of the agency, as well as tasks that are only performed during an emergency.

Evacuation Plan for County Health Department
This building evacuation plan included in this document may be used by all health departments and should be modified accordingly to fit the needs of your agency and county plans. This template is an optional item for health departments that do not have existing evacuation plans. If you have a current facility evacuation plan, it is not necessary to repeat it here but you may reference within the COOP that an evacuation plan exists and where to locate the plan.
