Data Requests

Most of the statistics prepared by the Public Health Informatics group are available online. However some tables are not reported online. If you would like to know if a table of statistics is available, please email Health Statistics. We will advise you if we have such a table available. If not you may need to make a data request.

If you are considering making a data request, please make sure the information is not already available in static (PDF) documents or reports or can be prepared by our online query tools - Kansas Information for Communities or Kansas Health Matters. If the information you seek is not available in a pre-existing report, a formal data request can be made by using the RD-1 Data Request Form and Data Request Guidance Packet (PDF).

Please note that if your data request involves programming and analyzing data, the requester may be required to pay certain fees. An estimate will be provided before the request is processed. The data request is also subject to denial or suppression of small numbers in order to comply with state law that prohibits identifying use of the data. Birth / Death Certificates

Do not use the RD-1 form if you are requesting a certified copy of a birth or death certificates. Those applications can be found at the Office of Vital Statistics.
