Ordering Certificates as a Walk-in Customer (Priority Service)
New ID Requirements
Due to the rise in identity theft and other fraudulent use of vital records, acceptable ID is limited. You must present a government (state or federal) issued photo ID. This can be issued by the U.S. or other country of residence.
Examples of acceptable IDs (must be current):
- Driver's license
- State ID card
- Passport or visa
- Military ID
If you do not have a government issued photo ID, you must bring any 2 of the following documents:
- Social Security number
- Bank statement with current address
- Car registration or title with current address
- Utility bill with current address
- Pay stub (must include your name, social security number plus name and address of business)
Costs & Availability
Note: Effective July 1, 2022, the cost of certified copies increased to $20 per copy. Prior $15 certified copy pricing expired at midnight on June 30, 2022.
Our walk-in Customer Service Lobby is open to the public from 9 am through 4 pm weekdays, excluding holidays. When you walk into the lobby, please proceed to the first available kiosk to request a certificate(s).
The cost is $20 for one certified copy and $20 for each additional copy of the same record ordered at the same time. Payment may be made by cash, check, money order, or credit card.
Office Location Details
The Office of Vital Statistics is located on the first floor of the Curtis State Office Building, which can only be accessed through the main entrance off 10th and Jackson Street. Jackson Street is a one-way street north. When you enter the front of the building, walk past the Security Desk to the pillars and go left down the hallway to our office. The office provides walk-in customer service from 9 am to 4 pm, Monday through Friday.
